The single most useful question
"Have you been around mice or in a closed cabin / shed / barn recently?" Severe muscle pain plus a rodent-exposure history are the combination that should put hantavirus on the differential — flu and COVID don't have that context.
What hantavirus looks like that flu does not
- Severe muscle pain in thighs, back, hips — sharper than typical flu myalgia.
- GI prominence: nausea, vomiting, abdominal pain, diarrhea early in the illness.
- No upper-respiratory symptoms in most cases — sore throat, runny nose and congestion are usually absent.
- Sudden cardiopulmonary collapse after a deceptively mild few days.
- Geography: western US (Sin Nombre) or southern South America (Andes).
Frequently asked questions
Hantavirus or flu — what are the early differences?
Both start with fever and aches; hantavirus tends to skip sore throat and runny nose, has more severe muscle pain in the thighs/back and pronounced GI symptoms.
Can hantavirus cause pneumonia?
It causes pulmonary edema (fluid in the lungs) rather than classic bacterial pneumonia. The chest X-ray pattern is ARDS, not lobar consolidation.
Hantavirus vs COVID-19 — which is worse?
Per case, hantavirus is roughly 50× more lethal. Per population, COVID has caused vastly more total deaths because it is far easier to catch.
Does hantavirus cause a sore throat?
Usually no. Sore throat strongly favors flu, COVID or strep over hantavirus.
Does hantavirus cause diarrhea?
Yes — GI symptoms including nausea, vomiting, abdominal pain and sometimes diarrhea are part of the prodromal phase.
